Lost/stolen and damaged passports

CONSULATE GENERAL OF INDIA,  JEDDAH

Requirements for passport in lieu of LOST/STOLEN passports

 

1  Applicants are required to fill online application form   at https://portal5.passportindia.gov.in and carry a printout of the filled   in application form. 

2  FORM-12- Affidavit for a passport in lieu of lost/damaged passports

3  Jawazat/Police Report for loss of passport alongwith its authorised   English legal translation.

4  Jawazat printout of Iqama details/residency status alongwith its   authorised English legal translation.

5  Two recent passport-size photographs of 2 inch x 2 inch with white   background. 

6  Photocopy of Passport (first two and last two pages and all pages of   observations, if any, made by Passport Issuing Authority)

7  Copy of valid Iqama (original to be shown for verification). 

8  Prescribed fees.

Personal presence of the applicant at the Application Centre is a must.

10 Tatkal facility (same day delivery) is available only in Jeddah (Hail Street   Jeddah) (Sun-Thu). for an additional charge of SAR 563/-.  PRIOR   APPROVAL FROM THE CONSULATE IS REQUIRED FOR TATKAL CASE   UNDER LOST CATEGORY.

 

Requirements for passport in lieu of DAMAGED passports

1  Applicants are required to fill online application form   at https://portal5.passportindia.gov.in and carry a printout of the filled   in application form. 

2  FORM-12- Affidavit for a passport in lieu of lost/damaged passports.

3  Two recent passport-size photographs of 2 inch x 2 inch with white   background. 

4  Original damaged Passport.

5  Photocopy of Passport (first two and last two pages and all pages of   observations, if any, made by Passport Issuing Authority)

6  Copy of valid Iqama (original to be shown for verification). 

7  Prescribed fees.
